Our Services
Vouch Orthotic Labs LLC fabricates custom foot orthotics and related devices for licensed healthcare providers and their practices. All devices are fabricated according to the prescription, scan, or impression provided by the ordering clinician. Vouch does not provide medical advice, diagnosis, or treatment. The ordering clinician is solely responsible for patient evaluation, prescription decisions, device selection, fitting, and patient care.
Professional Use Only
Our services are intended for licensed healthcare professionals and their practices. By submitting an order, you represent that you are, or are acting on behalf of, a licensed provider authorized to prescribe and dispense the devices ordered.
Age Requirement
You must be at least 18 years of age to use this site, submit a form, or opt in to receive text messages from Vouch Orthotic Labs LLC. By using our site or services, you represent that you are at least 18 years old.
Prohibited Activities
You agree not to engage in any prohibited activities while using our site or services. Prohibited activities include unlawful use of the site or services, misrepresenting your identity or professional licensure, submitting false or fraudulent orders, interfering with the operation or security of the site, and copying or harvesting content or data without authorization.
Termination
We reserve the right to suspend or terminate services without prior notice for any violation of these Terms.
Orders and Free Device Offers
Promotional offers, including free custom orthotic offers, are available to qualifying practices, are limited to quantities stated in the offer, and may be modified or withdrawn at any time. Turnaround times published on this site are averages, not guarantees. Pricing, remake, and return policies are provided with your account terms or upon request.
Text Messaging Terms
If you opt in to receive text messages from Vouch Orthotic Labs LLC, you agree to receive messages related to your inquiry, order updates, and occasional marketing or promotional offers at the phone number you provide. Message frequency may vary. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for assistance. Consent to receive text messages is not a condition of purchasing any goods or services. Carriers are not liable for delayed or undelivered messages. User Consent
Opt-In
By providing your mobile phone number to Vouch Orthotic Labs LLC, you voluntarily opt in to our messaging program and agree to receive recurring mobile messages from us at the number associated with your opt-in, even if that number is registered on any state or federal Do Not Call list. You agree that any mobile phone number you provide is a valid number of which you are the owner or authorized user. If you change your mobile phone number or are no longer the owner or authorized user of that number, you agree to promptly notify us at care@vouchorthoticlabs.com. Participation in the messaging program is not required to make any purchase from us and is completely voluntary.
Opt-Out and Support
You may opt out of the messaging program at any time by replying STOP, QUIT, CANCEL, OPT-OUT, or UNSUBSCRIBE to any mobile message from us. You may continue to receive messages for a short period while we process your request, and you may receive a one-time opt-out confirmation message. Limitation of Liability
To the maximum extent permitted by law, Vouch Orthotic Labs LLC is not liable for indirect, incidental, or consequential damages arising from use of this site or our services. Our total liability for any claim related to a device is limited to the amount paid for that device or its remake or replacement.
Indemnification
You agree to indemnify and hold harmless Vouch Orthotic Labs LLC from claims arising out of your practice's patient care decisions, including prescription, fitting, and clinical use of devices we fabricate.
Governing Law
These Terms are governed by the laws of the State of Delaware, without regard to conflict of law principles.
Changes to These Terms
We may update these Terms at any time without prior notice. Continued use of the site or our services after changes take effect constitutes acceptance of the updated Terms.
